What Is Perimenopause?
Perimenopause is the transitional phase before menopause — typically beginning in a woman’s early-to-mid 40s, though it can start earlier. During this time, estrogen and progesterone levels fluctuate unpredictably, causing a wide range of symptoms that can last anywhere from a few years to over a decade.
The Symptoms Most Women Suffer in Silence
Brain fog and memory lapses. Difficulty concentrating, forgetting words mid-sentence, feeling mentally “slow” — these are among the most distressing perimenopausal symptoms and among the least discussed. Estrogen plays a direct role in cognitive function, and its fluctuation directly affects how sharp you feel day to day.
Sleep disruption. Not just night sweats — but lying awake at 3am unable to fall back asleep, waking unrefreshed, or feeling exhausted despite adequate hours in bed. Poor sleep during perimenopause is driven by hormonal shifts that affect melatonin and cortisol regulation.
Mood changes and anxiety. Sudden irritability, low mood, heightened anxiety, or feeling emotionally “not yourself” are very common in perimenopause — and very commonly misdiagnosed or dismissed. These are hormonal, not psychological.
Vaginal dryness and painful intimacy. As estrogen declines, vaginal tissue becomes thinner, drier, and more fragile. Intimacy becomes uncomfortable or painful. This is known as genitourinary syndrome of menopause (GSM) and affects nearly half of women in the menopausal transition — yet fewer than 25% ever seek treatment.
Weight changes and metabolic shifts. Many women notice weight gain — particularly around the abdomen — during perimenopause even without changes in diet or exercise. This is directly driven by hormonal changes affecting insulin sensitivity, cortisol, and fat distribution.
Joint pain and fatigue. Estrogen has anti-inflammatory properties. As levels drop, joint aches, muscle tenderness, and persistent fatigue become common — symptoms often attributed to “getting older” rather than identified as hormonal.
What Actually Helps
